Community Paramedicine aims to revolutionize patient care by connecting individuals to high-quality medical services in the comfort of their homes. Trained community paramedics, working under the supervision of a physician, screen, assess, and treat patients who may not need emergency or hospital care. Leveraging telehealth services, patients and community paramedics connect with a physician in the Northwell Health Physician Partners network or NETS, ensuring medical expertise is readily available. This program expands access to care for homebound seniors and other vulnerable members of our community.

Caregivers Center: The Thomas E. and Alice Marie Hales Caregivers Center is led by a licensed master social worker and a team of trained volunteers to help ease the stress associated with caring for a loved one. The center features:

  • Emotional support and crisis intervention

  • Connection to resources, referrals to external support, and help navigating of advanced care planning and end of life

  • Facilitation of difficult conversations and family sessions

  • Monthly open-discussion Caregivers Support Group, educational workshops and community caregiver days

The 2-mile loop of the Walk will take participants through scenic routes including Rockwood Park, Kendal, and our beautiful campus.
